To help us provide the best care possible we need your help in always providing the following information:

  • Bring your insurance cards
  • Let us know if your address, telephone number, employer, marital status, primary care physician, or insurance changes.
  • Each visit, bring a list of all medications that you are currently taking, over-the-counter remedies, vitamins, and herbal supplements. On this list be specific on the drug, dose and frequency that you are taking the drug.
  • We want you to have a very active role in your care and to have all of your questions answered. To make the most of your visit write down questions or concerns that you may have.
  • If you wish, feel free to bring someone with you to your appointment. Having someone with you not only give your support but also provides someone else to listen to your physician's comments and responses to your questions.
